Programs and Affiliates

Children’s Health Fund (CHF) develops and supports a National Network of 22 programs and two affiliates in 15 different states and the District of Columbia. Seventeen of the programs/affiliates are based at academic medical centers, the other seven at Federally Qualified Health Centers.

Programs provide comprehensive health care through an enhanced medical home model to medically underserved children. This model integrates intensive primary care, mental health and oral health services through innovative practices, such as the use of mobile clinics, school-based clinics and telehealth.

Special Initiatives include: the Referral Management Initiative (RMI), the Childhood Asthma Initiative, the Transportation Initiative, Medical Home Initiative and Starting Right (Obesity Prevention).

The 22 programs and 2 affiliates provide services at more than 200 different sites, including those served by nearly 50 mobile clinics, as well as fixed site and school clinics. The majority (> 85%) of the sites serve solely pediatric (birth – 24) populations with the others providing family care.
