BRONX Health Collective

Bronx Health Collective (BHC), a program of Montefiore Medical Center and the flagship program of Children’s Health Fund, has served New York City’s (NYC) most marginalized children and families for over 30 years.

Bronx Health CollectiveBHC is located in the poorest Congressional district in the nation and serves some of the city’s most vulnerable populations, including children and families living in the South Bronx and those living in NYC’s shelter system. BHC serves these medically underserved populations through a network of community health centers, mobile medical units, and part-time clinics in the shelter system.

BHC’s medical home model of care is designed to address the complex health and psychosocial needs of vulnerable families, offering comprehensive access to primary care across the lifespan, with co-located and integrated mental health, case management, primary care, and nutrition services. In addition to comprehensive clinical care, BHC provides integrated perinatal and family planning services, substance use disorder services, and a medical-legal partnership for newly arrived immigrant children and families.

Care is made accessible through bilingual providers and real-time translation services, transportation assistance, and low-cost prescription medication.
