TRENTON Children's Health Project

The Trenton Children’s Health Project (TCHP), a partnership between Henry J. Austin Health Center (HJAHC) and Children’s Health Fund (CHF), was launched in 2019 to provide quality healthcare for communities in underserved areas of greater Trenton, New Jersey.

Trenton ProgramThe leading healthcare center in Mercer County, HJAHC reaches about one-third of Trenton’s population, of which more than 30% are children. By integrating primary care, mental health, and substance abuse services, TCHP effectively serves patients with multiple needs, leading to improved outcomes. TCHP serves all Mercer County residents regardless of whether they have health insurance. They can receive comprehensive and quality primary care, pediatrics, women’s health, behavioral health, dental, vision, pharmacy, and podiatry services.

Clinical pharmacists embedded within healthcare teams make recommendations on appropriate therapy. Integrated care sites also ensure that community members have access to mental health services, and improved access to primary care.

To reach children in neighboring communities, TCHP plans to launch a mobile health program in partnership with the local school system.
